The advantages of minimally invasive (laparoscopic and robotic) surgery compared to open (classical) surgery are predominantly related to a shorter patient recovery and a faster return to daily activities. Also, less blood loss, a reduced degree of postoperative pain and a better aesthetic result (smaller incisions on the abdominal wall) have been recorded after minimally invasive operations. We should not forget the superior visualization of structures and more precise surgical work, with the help of a sophisticated optical system (i.e. a camera that is introduced into the abdominal cavity) and modern instruments - which is especially important in robotic-assisted surgery. However, there are diseases and conditions that make open surgery a safer and more secure option for the patient. Patients with uncontrolled underlying health problems (primarily heart and lung diseases) are often candidates for open surgery, due to the potential risks that minimally invasive surgery technology may pose to the functioning of vital organs. Also, adhesions due to previous abdominal surgeries or the characteristics of the tumor process itself (e.g. tumor size or tumor growth into surrounding structures) may pose difficulties in performing a minimally invasive procedure.
